A workable editorial workflow has five states: idea, drafting, review, approved, and published. Anything more granular tends to get ignored, and anything less makes it impossible to see where work is stuck.
Each state needs one owner. The most common failure is a draft sitting in review with nobody specifically responsible for moving it forward.
